Where are the pre-registration forms?

OK , sometimes I can be a little dense. Where are the "pre-registrations" forms located on the site? I cannot seem to find them.

The pre-registration form is used for driving schools. Upper left under Navigation.

The DIY forms will be viewable when registration opens up.
